The probablity of failure (POF) is how likely an asset is to fail. The consequence of failure (COF) indicates how serious the consequence is if the asset does fail. Both are ranked on a scale from 1 to 5. This chart clearly defines all rankings from 1-5 and can be used as a reference when determining an asset’s POF and COF.
